Rates:
$100 per 50 minute session. Cash and check are accepted.
Fees may be reduced for those with extraordinary
medical expenses or other financial hardships, and will be determined
on an individual basis. Please inquire if you wish to be considered
for this reduction.
Insurance:
I am a participating provider with Anthem Blue Cross and Blue
Shield, and Aetna.
Services may also be fully or partially covered
if you have out-of-network coverage with another insurance company.
This means that you would be responsible for the fee at the
beginning of each session and then you would submit a receipt
that I prepare to the insurance company for reimbursement. You
can check with your insurance company to verify your benefits
if you wish to be reimbursed or, with your permission, I can
inquire for you.

If you choose to use in-network or out-of-network insurance coverage, please remember that your insurance carrier may require that you authorize the release of confidential information.
Cancellation/Missed Appointments
Policy:
24-hour notice is required for cancellations. If less than 24
hours is given, the full fee will be charged, unless an appointment
can be re-scheduled during the same week or the cancellation
is due to illness, emergency or a planned vacation. Cancellations
will be accepted in person or by telephone only.
Privacy Policies:
This office is HIPPA compliant. All efforts will be made to
respect and protect your confidentiality.
